Please find below our boarding agreement for your dog. I would appreciate it if you could sign it and either fax, post or Email it back

Our hours are 6.30am to 10.00am and 4.pm to 6.30pm daily. No exceptions without prior agreement (after hours fee applies min. $10). We charge $1 a day to administer medications and $1 a day to store and serve a dog's own food. Over 1/2 hour late without notification = $10.00 late fee.

Our kennels are Hastings District Council approved.

Elizabeth and Max Patmoy offer a highly ethical, caring and safe environment for dogs. This includes individual accommodation, quality diet, spacious exercise areas, optional walks and stringent health and hygiene management. Please note: NO VACCINATION (including kennel cough) = NO BOARDING!!! Do bring some bedding for your dog but, please,  no bedding with polystyrene beads.

We charge in half days ($9). Ideally a dog would stay between 6.30 to 10am on one day and leave between 4pm to 6.30pm on another day. The cost would be $18 a day. If the dog's stay is in excess of a "day" period but less than an extra day, e.g. drop off is between 6.30 to 10am Friday and uplift between 6.30 to 10am Saturday the charge is one and a half days.

You agree to the following:- 

1.  My dog/s are fully vaccinated against parvovirus, hepatitis, kennel cough, and distemper and have an IMMUNITY TIME FRAME ALLOWED of 72 hours.

2.  I give the proprietors full authority to seek veterinary attention should they deem it necessary and/or any anesthetic  or surgery, including transport costs. Any veterinary account whatsoever will be my responsibility.

3.  I have provided information concerning special problems or behavior of my animal/s and allergies or medication that may pose a risk to my pet or others. This includes biting, aggression climbing, digging and escaping. I undertake to advise the proprietors of any changes to the information contained in this agreement, prior to the commencement of any boarding period.

4.  I acknowledge that the proprietors and their employees cannot be held responsible for injury, death, loss or damage etc. of any kind whatsoever that may occur to or be caused by my animals.

5.  In the unlikelihood of death, I give permission to arrange an autopsy to ascertain the cause of death, including the possibility of infectious disease.

6.  I understand the proprietors accept no financial responsibility for collars, leads, bedding, toys, cages etc.

7.  I agree that in the event of arrears or after due notice in writing, the proprietors shall have the right to dispose of the animal/s and this shall not release me from liability for any unpaid final charges.

8.  I agree to discuss special facilities and/or prices for my female dog (bitch) whilst in season.

9.  I understand the proprietors reserve the right to charge for facilities reserved for all days, including unused days and cancellations made by me at any time without 48 hours notice.

I agree that it is my responsibility to produce a current vaccination certificate at reception and to adhere to hours of attendance, terms of trade and terms of payment.
